At Medford Care Center, Subacute Care is an alternative that combines quality of life with quality of care. Subacute care is offered to patients requiring transitional care following an inpatient hospital stay. We offer a unique clinical setting that more readily accommodates a patient's personal needs. Because we combine the finest levels of care with the comforts of home, patient and family needs are better met. Advanced medical technology and tastefully furnished rooms are the mainstay for the subacute units, which include electric beds, and a highly trained staff make this the finest subacute facility in central New Jersey. The facility contracts with all major managed care and insurance companies and has professional affiliations with all of the major area hospitals and medical centers. Medford Care Center provides rehabilitation following joint replacements and fractures. We also offers pre-operative classes prior to joint replacement surgery. We offer a fulltime staff of social workers to assist you with your discharge to home. Our staff will arrange your visiting nurse services, home rehabilitation, outpatient rehab and order any equipment you need before returning home. We will also set up any community programs necessary for your return home.

TRANSITIONAL MEDICAL SERVICES
Provide medically complex patients with acute needs delivered by acutecare trained nurses and rehabilitation therapists. Patient and family education is custom tailored to promote independence in the home environment.

SKILLED NURSING SERVICES
Provide short-term medical management and skilled nursing intervention as short-term alternatives to acute hospitalization or home care.

OVERVIEW OF SUBACUTE SERVICE AND STAFF
Medford Care Center offers comprehensive programs which focus on the total needs of each patient.

All patients will receive the following basic services:

  • 24-hours physician coverage, access to medical specialists
  • RN supervision 24 hours a day
  • Clinical case management
  • Physical Therapy, occupational therapy, speech and language pathology as needed 7 days a week, twice per day if indicated

REHABILITATION SERVICES:

Rehabiltation Therapy Program
